Navigation

Select the Reports tab in the navigation bar
Select Standard Reports
In the Select Report section, locate Lost Time Report from the drop down

This report layout is displayed in 1 section;

A table that includes;

2. Incident Date
3. Org Unit
4. Entity
5. Person Involved
6. Classifications
7. Month Reported
8. Type
9. Number of Days / Shifts
10. Ongoing
