About Anonymous Questionnaires

Anonymous Questionnaires cannot form part of Corporate Compliance because there is no known person to fulfil the Corporate Compliance requirements.

Additional publishing options, along with publishing and reporting restrictions also apply to anonymous Questionnaires to help maintain recipients’ and respondents’ anonymity.

Publishing (Distribution) Options

There are three publishing options available to Questionnaire publishers in PolicyHub, the Standard option and two anonymous options.

  • Standard (Not anonymous)
  • Anonymous
  • Optionally anonymous.

Standard (Not Anonymous)

With the standard option, recipients respond to a Questionnaire with no anonymity – that is, all of their questions and answers can be viewed and reported on fully.

This does not affect any existing reporting functionality and it can be used as a part of the Corporate Compliance requirement.

Anonymous

With this option, Questionnaire publishers can select (at publication setup) to distribute

Questionnaires which can be answered anonymously by recipients.

Upon receipt of an anonymous Questionnaire, the recipient can choose to not respond to the Questionnaire by rejecting it, or ignoring it and letting it time out.

Upon completion of an anonymous Questionnaire, the respondent can only submit the Questionnaire by selecting the Submit Anonymously button.

  • This affects the reporting of Questionnaires, because, anonymously submitted questions and answers are stored anonymously and the act of rejecting is also stored anonymously. Reporting cannot differentiate between an anonymous submission and a rejection.
  • The respondent will not be able to Save and Exit the Questionnaire as this requires saving the questions and answers to the database.

Optionally Anonymous

With this option, Questionnaire publishers can select (at publication setup) to distribute

Questionnaires which can be answered anonymously or not, by recipients.

Upon receipt of an optionally anonymous Questionnaire, the recipient can choose to not respond to the Questionnaire by rejecting it, or ignoring it and letting it time out.

Upon completion of an optionally anonymous Questionnaire, the respondent can submit the Questionnaire by selecting the Submit Anonymously button or the Submit button.

  • This affects the reporting of Questionnaires, because, anonymously submitted questions and answers are stored anonymously and the act of rejecting is also stored anonymously. Reporting cannot differentiate between an anonymous submission and a rejection.
  • The respondent will not be able to Save and Exit the Questionnaire, as this requires saving the questions and answers to the database before they have chosen whether or not to submit the Questionnaire anonymously.

Publishing (Distribution) Restrictions

Various publishing restrictions have been included in PolicyHub to ensure recipients’ anonymity is maintained.

Restricted Publishing Functionality

To maintain a recipient’s anonymity:

  • Anonymous and Optionally Anonymous Questionnaires cannot be published to one recipient.
  • Anonymous and Optionally Anonymous Questionnaires cannot be published to a group with only one member.

Reporting Restrictions

Various reporting restrictions have been included in PolicyHub to ensure recipients’ anonymity is maintained.

Restricted Reporting Functionality

To maintain a recipient’s anonymity:

  • Anonymous Questionnaires cannot form part of Corporate Compliance reporting, because, as the Questionnaires are anonymous, there can only be reporting on questions and answers.
  • Compliance and audit reports will not be available for any publication submitted using the anonymous or optionally anonymous options.
  • The Export to CSV functionality will only list the individual anonymous Questionnaire responses, the respondents will be listed as Anonymous.
  • The Questionnaire summary report will not be able to report on any anonymous Questionnaire responses for a single respondent.
  • The CSV export report will not be able to export any anonymous Questionnaire responses for a single respondent.

Guaranteeing Anonymity

All publishing and reporting restrictions, in PolicyHub’s anonymous Questionnaire functionality, are included to ensure all recipients’ anonymity is maintained.

Terms And Conditions

Information or statements given in this document concerning the suitability, capacity, or performance of the mentioned anonymous questionnaire functionality cannot be considered binding. However, Hitec (Laboratories) Ltd have made all reasonable efforts to ensure that the anonymous questionnaire functionality is adequate and free of material errors and omissions. Hitec (Laboratories) Ltd will, if necessary, explain issues which may not be covered by this document.

The anonymous questionnaire functionality has been prepared to be used by professional and properly trained personnel and the customer assumes full responsibility when using it.

Hitec (Laboratories) Ltd welcomes customer comments as part of the process of continuous development and improvement of its anonymous questionnaire functionality.

Hitec (Laboratories) Ltd’s liability for any errors in this anonymous questionnaire functionality is limited to the documentary correction of errors.

In no event will Hitec (Laboratories) Ltd be liable for any damages including, without limitation, indirect or consequential damages, or any damages whatsoever arising from use or loss of use of information supplied through or obtained by the manipulation of this questionnaire functionality ether in action of contract, negligence or other tortious action.

Nothing contained herein shall be construed as conferring by implication, estoppel or otherwise any licence or right under any agreement to use this anonymous questionnaire functionality except for the purpose it has been specifically intended. If there is any conflict between these terms and conditions and/or specific terms of use relating to anonymous questionnaire functionality, the latter shall prevail.

If any of these terms and conditions should be determined to be illegal, invalid or otherwise unenforceable by reason of the laws of any state or country in which these terms and conditions are intended to be effective, then to the extent and within the jurisdiction which that term or condition is illegal, invalid or unenforceable, it shall be severed and deleted from this clause and the remaining terms and conditions shall survive, remain in full force and effect and continue to be binding and enforceable.

These terms and conditions shall be governed by and construed in accordance with the laws of England and Wales.

Disputes arising here from shall be exclusively subject to the jurisdiction of the courts of England and Wales.
