How to add notes to a questionnaire

If you wish to provide additional instructions or important notes to your client while they answer the questionnaire for their specific case, here's how you can do it.

1. First, navigate to Setup > Knowledge base > Questionnaire Template. You may search for the questionnaire using the search button as shown on the image below.


2. Enter the name of the relevant questionnaire on the field and click Find. Then click on the title of the questionnaire.


3. On the next page, click "Subsections".


4. Click the Note button next to the subsection you wish add a note.


5. On the pop-up window, enter your note on the dialog box then hit Save.


6. Once, you are back on the subsections page, hit Save again to save your changes. Once done, your client will see your note as shown on the image below.
